Access to medical files

In accordance with the law of 22 August 2002 on ‘Patients’ Rights’, as a patient you may obtain, with certain exceptions*, a copy of your medical records within 15 days of receipt of the request.

Please note that your medical documents, including laboratory results, radiology protocols and hospitalisation reports, are available online via the My Health website. Log in easily to view and download your results securely.

Request a copy of the file

To obtain their medical records, patients must submit a written request using the request form, duly completed and signed.
To be valid, the request must be signed and include a copy of the patient's identity card (front and back).

The right may also be exercised by a third party acting on behalf of the patient: either the trusted support person designated by the patient for this purpose, or the patient's legal representative (father, mother, guardian) if the patient is a minor and/or incapable of exercising his or her rights personally.

This third party is designated by means of a proxy.

This must contain all the details of the patient and the representative.

  • A copy of the file will be sent to you by post.
  • Issued on site at the St-Anne St-Remi Clinic reception desk (if the request has been made in advance and validated).

If the request concerns a deceased patient, the law grants a right of consultation to the next of kin through a professional practitioner designated by the requestor.

For such a request to be honoured by the hospital, the patient must not have objected during his or her lifetime and the request must be sufficiently substantiated.

* Exceptions

*ex: (article 9§3 of the law of 22 August 2002) : The professional practitioner may refuse to issue a copy of the medical record if he has clear indications that the patient is being pressured to communicate a copy of his record to third parties.
